Twilight Psychology is excited to announce that we now offer medical marijuana consultations and renewals for eligible clients in Kentucky. Our Authorized Practitioner, Nicola Allen, MSN (PMHNP), is currently accepting new patients with immediate availability.
About Medical Marijuana Consultations
Medical marijuana consultations are clinical evaluations to determine if a cannabis-based treatment plan is appropriate for your medical condition. Renewals are follow-up visits to assess ongoing treatment efficacy and adjust recommendations as needed.
What Medical Marijuana Does
Medical marijuana contains cannabinoids (primarily THC and CBD) that interact with your body’s endocannabinoid system. Depending on the strain and dosage, medical marijuana may:
- Reduce chronic pain by modulating pain signals
- Decrease inflammation associated with conditions like MS
- Alleviate anxiety and hyperarousal common in PTSD
- Support symptom management for cancer patients, including pain, nausea, and loss of appetite
- Improve sleep quality in many patients
The effects vary widely based on the cannabinoid profile (THC/CBD ratio), delivery method (edibles vs tinctures), dosage, and individual body chemistry. Our clinician will work with you to identify the optimal approach for your specific condition and symptoms.
Medical Marijuana and ADHD Considerations
While not currently listed as a qualifying condition under Kentucky’s medical marijuana program, ADHD and cannabis use require careful consideration. Some evidence suggests cannabis may temporarily reduce hyperactivity or impulsivity, but research also indicates potential risks:
- Cognitive impact: Regular use may affect working memory and focus—which are already challenged in ADHD
- Dependency potential: Individuals with ADHD may have higher risk of cannabis misuse or dependency
- Interaction with medications: Cannabis may interact with ADHD medications or other psychiatric treatments
If you have ADHD and are interested in medical marijuana for a co-occurring qualifying condition (e.g., PTSD, chronic pain), this will be discussed thoroughly during your consultation to ensure safety and appropriateness.
Potential Consequences & Important Considerations
Employment & Drug Testing
- Federal employment: Federal employers have strict cannabis policies; employees may be subject to termination regardless of state legalization
- Private employment: Many employers still conduct drug screenings; positive results may affect employment regardless of medical marijuana status in Kentucky
- Safety-sensitive positions: Positions involving heavy machinery, transportation, or security may have zero-tolerance policies
- Testing detection: THC can remain detectable in standard drug tests for days to weeks depending on use frequency and testing method
- Smoking THC: Smoking medical marijuana is illegal in Kentucky
Federal Legal Status
- Federal law vs. state law: Cannabis remains a Schedule I controlled substance at the federal level, despite Kentucky’s medical marijuana program
- Interstate travel: Transporting medical marijuana across state lines is illegal federally
- Military personnel: Federal law prohibits military members from using cannabis, even if recommended at the state level
Second Amendment & Gun Ownership
- Federal prohibition: Federal law prohibits any person who is a “user of a controlled substance” from possessing firearms or ammunition (18 U.S.C. § 922)
- Medical marijuana users: Even in legal states, individuals with active medical marijuana cards or who use cannabis may be prohibited from purchasing, possessing, or carrying firearms
- Background checks: Federal background check forms (ATF Form 4473) ask about controlled substance use; answering affirmatively may result in denial of firearm purchases
- Existing firearms: Individuals who already own firearms should consult with a legal attorney before beginning medical marijuana, as possession of both may create legal exposure

Other Potential Side Effects & Risks
- Dizziness or dry mouth from smoking or vaping
- Impaired coordination or reaction time — do not drive or operate machinery while impaired
- Anxiety or paranoia (particularly with high-THC strains)
- Dependency potential, especially with frequent or high-dose use
- Cannabis Hyperemesis Syndrome (excessive vomiting) in chronic heavy users
- Mental health considerations: History of psychosis or schizophrenia may increase risk with THC-heavy products
Eligibility & Requirements
Qualifying Medical Conditions
Medical marijuana consultations are available for clients with the following six diagnosed conditions:
- Cancer
- Chronic or severe pain
- Epilepsy or seizure disorder
- Multiple Sclerosis (MS)
- Chronic nausea or cyclical vomiting syndrome
- Post-Traumatic Stress Disorder (PTSD)
Geographic & Residency Requirements
- Clients must be located in Kentucky following the state’s legalization of medical marijuana
Clinical Evaluation
A clinical evaluation is required and submitted to the state irrespective of your client status with Twilight Psychology. This in-person assessment allows us to:
- Review your medical history and current diagnoses
- Discuss your symptoms and treatment goals
- Determine whether medical marijuana is an appropriate treatment option
- Provide guidance on dosing, delivery methods, and safety considerations
Existing Twilight Psychology client’s clinical history will be shared with Nicola and reviewed for medical necessity before approvals.
What to Expect During Your Consultation
During your evaluation with Nicola Allen, you’ll discuss:
- Your medical condition(s) and current symptom severity
- Current medications and potential drug interactions
- Your lifestyle, including employment and safety-sensitive responsibilities
- Any concerns about drug testing, firearms, or legal implications
- Your preferences regarding delivery method and cannabinoid profile (THC:CBD ratio)
- A realistic timeline for symptom improvement and follow-up assessment
Come prepared with questions. This is your opportunity to address all concerns about efficacy, side effects, federal implications, and how medical marijuana fits into your overall treatment plan.
